Issue results are worked off of numerous factors to determine what you see in the end. If every option did the exact same thing to every nation that chose it, it wouldn't be realistic nor very interesting. As for your particular result, note that this thread exists for reporting odd issue results. Could you provide the issue number and option you chose so we can better understand what happened?

It sounds very much like #26.1 which results in the enforced "donation" of organs as described.

Based on the nation's happenings, it looks like it's actually #728 4.

Which is a tradeoff between civil and economic rights, so I'm guessing that economic rights increased more than civil rights decreased. (As opposed to #26, which can be better read as a tradeoff between different civil rights - if you consider access to good medical care a civil right.)

True, some issue results are, so to say, counterintuitive. For instance, when I chose to deregulate airlines, it somehow increased taxation (significantly, I must say). Common sense dictates that the result must have been exactly the contrary. How can it be logically explained?
