The year is 2033, thirteen years after Patient Zero, and the world is unrecognizable.
The Chinese government's top levels had been planning for a controlled apocalypse since the Cold War. For years, the officials believed that the only way to win a global nuclear war was a massive population. When the USA and USSR had destroyed each other in a nuclear holocaust, millions of Chinese would overwhelm the small fragments of the rest of the world and the Middle Kingdom would reign supreme. However, when the Cold War ended, the probability of nuclear war fell, and the Chinese needed a new idea.
In 2018, the Chinese government sent a secret order to their biological labs to begin a project codenamed "The Final Phase." Their scientists had identified an obscure gene that only roughly 10% of the human population has in their genome, and then proceeded to create a biological weapon that only those with that gene are immune to. Finished in January of 2020, this biological weapon - now referred to as CB-001 - was never meant to be anything but an extreme last resort. No Chinese party member in their right mind would release such a disease upon the world: after all, many of them lacked the immunity gene. At first, this disease was meant as a sort of revenge on any foreign invaders. Should nuclear devastation destroy the Earth or invaders collapse the People's Republic, this disease would ensure that China's legacy would be remembered. China kept this virus under lock and key (and piles of classified bureaucratic files).
In March 2020, the Second Tibetan Revolution broke out. Tired of their occupation and oppression under Beijing, Tibetan revolutionaries stormed government offices, army bases, and Chinese settlements. China responded harshly, sending massive armies of troops, tanks, and artillery to fight Tibet, along with their Air Force. India and the US soon became involved in supporting the rebels (Albeit surreptitiously), giving the Tibetan people a fighting chance. Lhasa became the central point in the conflict, leading to hundreds of thousands dying in this city alone. For the next month, Tibet burned.
Among the casualties of this conflict was a small laboratory deep in the plateau, simply named IU-7. The Tibetan rebels flooded into the lab, pillaging and taking useful items to aid their cause. They quickly left upon finding little of use, but not before a single vial of CB-001 crashed to the floor, infecting a lieutenant. Lieutenant Dorje continued travelling with his crew, before falling sick with an unknown disease. He was transported to the central hospital in Lhasa, infecting the doctor. This doctor, Dawa, had the gene which allows him immunity, and so became a carrier. Over the next two years, the disease spread throughout the Tibetan rebels, crippling their rebellion. The Chinese soldiers easily put it down, then contracting the disease themselves and spreading it to their home towns as they retired or were brought to hospitals.
By late 2022, the disease had become widespread in Beijing, Hong Kong, Shanghai, and numerous other cities on the Chinese mainland. The disease had become a full-blown epidemic, but had failed to get much media coverage due to coverups by the government.
On the 4th of December 2022 the first case of CB-001 (Now being nicknamed "Chinese Fever") was reported in Tokyo, Japan. On the 5th, the first case in India was reported in Vijayanagar. On the 7th, the disease had been reported in Honolulu, USA and Barcelona, Spain.
By April 2023, the disease was present and spreading in much of Europe, Africa, and Latin America. By this point, governments throughout the world began to take action. Medical Research companies were directed towards cure research, the US and UK declared a joint state of emergency, medical care throughout the world was focused almost entirely on containing the threat. By September, the death toll passed 150 million. The infection rate neared 200 million. The Great Panic began:
The EU forbade any travel between its states without intensive medical screening. The US quarantined New York City and Los Angeles, Japan quarantined Kyoto and Tokyo, Israel declared it would kill any Illegal immigrants on sight, China bombed the Lhasa Quarantine Zone, and Burma declared a "national medical screening", proceeding then to gas entire villages in an attempt to kill the infected. However, the worst had yet to come. Americans began to flood Canada in desperate hope to escape death's looming veil, while the Chinese and Russians flooded Siberia and Europe flooded Scandinavia. Ill equipped to deal with the harsh weather, millions died from exposure, starvation, and hypothermia in the winter of 2023-2024. Meanwhile, the situation continued to worsen elsewhere. The lack of health infrastructure and organized responses let Africa become the fastest-growing hot zone, with the entire continent becoming infested within months. Two Pakistani pilots, immune to the disease and vowing revenge on the "Hindu bastards," dropped three nuclear weapons on Delhi, Calcutta, and Bombay, killing many survivors of the plague. In China, the Three Gorges Dam burst and floods the still densely-populated river valley, killing millions and destroying hundreds of miles of valuable farmland.
The number of infected passed 1 Billion by July, 2024, soon passing 2 billion and 3 billion in the following year. The 90% death rate was close behind, killing the infected within weeks of initial exposure. By January 1st, 2026, 600,000,000 humans were left alive on Earth. 6 Billion had been killed by the virus, with another 400,000,000 killed by the panic. However, the remaining scientists estimate that nearly every human left is immune. Apart from certain worrying cases in which supposedly immune individuals became infected, the disease is under containment.
Iceland and Greenland are the only completely uninfected areas, having gone into a state of self-quarantine. They are currently completely cut off from the outside world and trade only with each other. A political merger is likely. Unfortunately, the successful quarantine and isolation had an unforeseen side effect - 90% of the population is still not immune to the disease.
All other national governments fell. Megacities reduced to cities, cities reduced to towns, and towns reduced to almost nothing. In most nations, warlords took over their immediate areas, creating micro-nations rife with chaos. In others, survivors banded together to form new governments. It is now seven years after the disease stopped spreading, and these new nations are finally getting off the ground and reclaiming territory from the warlords.

-Population for your nation can be calculated by taking 10% of the current population, then subtracting any disasters. India and China were hit with especially bad disasters to try to even out the populations. You can also add population through migration from other regions- if your government was founded early, you may have gotten migrants from nearby warlord provinces. Be sure to explain this, though.
