The implications of Caesar as an analog to Kurtz really are tremendous, though. I mean think about it...

Pretty major spoilers for Heart of Darkness, Apocalypse Now, and Fallout: New Vegas ahead, so I guess don't read it if for some reason you're unfamiliar with the resolutions to those stories.

Conrad's Kurtz was created to essentially prove to affluent 1890s Europeans that Colonialism, which was making them all very rich and very happy, was such an extremely evil thing that they should all immediately cease all their colonization activities and may god have mercy on their souls.
He dies at the end of HoD to prove just how corrosive the horror of the human heart truly is - even when it appears to be doing good, even when it appears like the future will be nice, humanity comes in just to sweep it out yet again. The idea was that the human psyche is so evil, is so completely beyond saving that any meager attempt at repentance will be quickly extinguished.
Coppola's Kurtz (which must be discussed, as Apocalypse Now's portrayal of the character is perhaps more profound even than HoD's), on the other hand, is already dying by the time he is seen in the story - in every possible way. He's portrayed as a dreamer who cannot sleep, an artist who has lost his touch. The world's greatest general but with no target. He had malaria, but was so emotionally destroyed by what he was being forced to do by the world that he wouldn't let himself die from it just out of spite. In a way, this Kurtz goes deeper than Conrad's - it's not a story about the evils of humanity, but about just how far the 'balance' of the universe can be bent before it is broken.

How does this relate to Caesar?

Well, it basically is Caesar's story. Caesar is a character that, in-universe, by all accounts really shouldn't exist. He's a harbinger of the old world system. He employs methods that are outdated even by pre-war standards, he's an intellectual/idealist surrounded by swathes of brutish lunks, and his goals are completely different from every other character in the game, trying to find his City of the World's Desire in the middle of a fucking desert.
And everyone wants him dead.
I mean literally everyone. He's the big bad unless you're working for him. Even his fucking commanders are waiting for him to die so they can take the reigns. Even the tribes he conquered want him dead, they just want other people dead more.
And he's dying.
You know I never really understood that schtick. The factions in FNV were all so perfectly painted to have a fatal flaw that equally counteracts their strengths, but the Legion's just seemed so weak. I mean, "One man can change the world - but what if that man is dying?" is such a lame-ass plot idea it's almost offensive to the beauty of the rest of the game.
But now I get it.

The brain tumor - it was metaphorical.
Sure Caesar is literally dying. But the tumor isn't going to kill him, just like malaria wasn't going to kill Colonel Kurtz.
The tumor was representative of Caesar's intellectual death - he had broken with himself. His ideals of what the wasteland should be were killing him. He knew what he had to do, and he was absolutely not prepared to do it. But he was going to anyway.

And so by the time of the events of the game itself, Caesar is more or less done. He knows that the cards are stacked against him. He knows that there is literally nobody else alive on Earth who will carry out his vision after he dies. So he doesn't want that.

I think this is why the Legion storyline is so half-assed. There really isn't supposed to be a Legion storyline at all. Caesar is supposed to be Kurtz, through and through. And as part of that, three things should be true for Caesar's story to be complete:

1. You have to spend a really long-ass time learning about him and getting opinions about him before you see him - in other words it's probably best not to go to the Fort until the NCR or House or whatnot sends you there.
2. You have to listen to pretty much everything Caesar has to say - let him tell his story to someone as intellectually capable as himself, because it's not like he'll ever get the chance again.
3. Caesar has to die before you leave the Fort the first time.

With all three of those things true, Caesar's story is almost exactly the same as Kurtz's, and New Vegas's larger plotline (that is, of the Legion against House against the NCR) can continue without Caesar's metaphorical tumor growing inside it.
