Yes. They get one final appeal. It is reviewed by a team of uninvolved mods who reexamine all the evidence and either accept or deny it. There are no further chances after that if the appeal is denied.

So, that's that after that? I remember some mod, may have been Reppy, saying that they may let DOS nations come back if they haven't been seen for a few years, and come back quietly and show no signs of the rule breaking behaviour that got them banned? Though this is completely separate to the appeals process of course.
