OB/GYN Division:
33 yr old caucasian female from Hauras presents at 18 weeks gestation complaining of vaginal bleeding with thick clots of blood. She is also having abdominal cramping. Her BP is 111/70, HR is 90 and RR 30. She is anxious of fetal loss. Ultrasound was performed revealing a crown rump of 5mm without any cardiac activity. Her bHCG was also very low at 2,000. She was given 800 mcg Misoprostol and 200 mg of Mifepristone. Further investigation revealed she has trisomy 16 abnormality.
MSK and Rheum Cooperation Committee (MRCC)
A 55 yr old AA female presents to the clinic with 6 weeks of bilateral wrist pain. Her temperature is 98 and skin has a warm feel to it. Her vitals are normal. Radiographs revealed diffuse carpal pannus formation with equal joint space narrowing. She has also been having numbness and tingling in her thumb, index and middle finger. This is likely due to inflammation within the carpal tunnel secondary to RA. She is placed on Methotrexate 8 mg per week. Dr. Hauss, Rheumatologist and Researcher in Inflammatory markers
Cardiac Service and Research Institute Intl:
66 yr old caucasian female presents with nausea and vomiting. She has limited history as she is homeless and a foreigner. Initial EKG did not show any significant changes but her troponins were extremely elevated. She was given nitrate sublingual 0.4 mg which helped relieve her symptoms. Started on 50 mg IV Metoprolol but she suddenly had bronchospasm so was switched to CCB Verapamil. Transferred to the ICU within the main hospital. TIMI score could not be calculated as enough information was not present. She was given Clopidogrel 300mg and ASA 325 mg for initial cath protocol.
Cath results indicated 40% blockage and severe 60% blockage of the LAD and RCA respectively. Post Cath she will be staying with the Cardiac Unit on 80 mg Atorvastatin and close monitoring of BP and medications.
