Universal Health Care in your Nation

Postby Moralem Populi » Tue Jul 16, 2013 12:50 pm

Tell whether your nation has a universal health care system, and if it does, describe which system it has (ex: socialized, single-payer, compulsory private insurance, two-tier, etc).
If your nation does not have a system, describe what system you follow as well (ex: completely free-market, market with regulations, etc).

The Constitutional Monarchy of Moralem Populi has a single-payer health care system, with socialized insurance and private practice. All citizens and legal immigrants are insured under this system, no exceptions.

Do we look like communists? We have a fully privatized healthcare system; the state only sometimes provides vouchers to the poor with which private healthcare providers can be paid to make sure even the poorest have the access to emergency care, but other than that, it's all private.

Our health care is so government-controlled and universal that we have our own Health Police - nurses that patrol looking for people they suspect of having an injury.

Single-Payer Healthcare Plan. All hospitals are private, save for community or charity hospitals. However, there is a national tax that all people must pay (whether enrolled in Healthcare plan or not). Those enrolled in the National Healthcare Plan must pay a premium every time one goes to the hospital but the Premium is incredibly low ($3, $4, etc.).

They must also pay a static premium every month or biweekly depending on how their employers pay them. It is the ONLY tax in Ulvena where the rate of tax is linearly increasing depending on income, with a maximum cap at $10,000 per year. There is an opt out for those who have contributed significantly to Ulvena or they are too poor to afford to pay the premium. It is an incredibly rare circumstance.

This combination of National Healthcare and privatized hospitals mean the traditional socialized healthcare system's method of waiting long times is eliminated, as is the pitfalls of completely privatized healthcare. That isn't to say we don't have privatized healthcare, which covers a far wider array of things, including cosmetic surgery, non-emergency abortions, and homosexual therapy.

Most Kiravian states (the exceptions being Arolinea and Daridia) have programs to pay for the basic and emergency medical needs of the poor and other disadvantaged or remote populations. Many states have more extensive programs that pay for a greater range of procedures and/or cover middle-income people as well. Only Sânctifrancisca, Sorvrina, Kesta, and Erinava have all-encompassing single-payer systems.

There are a few corporations that sell óprosciunar (best translated as "come-uppance insurance") to the poor, charging very small monthly fees in return for promising to cover the costs of important procedures that aren't covered by a particular state's system for one reason or another. A few states have an appeals process that allows patients seeking coverage for procedures not normally covered by the state to convince a panel of doctors of the procedure's necessity.

The Caritist political faction, who are religiously-inspired socialists and distributists, advocate for the expansion of socialised healthcare, and are gaining influence in both state and Federal legislatures.

Healthcare is far from universal in Aelbeos. Most rural communities will have wise men and women healers, and the local priest is always likely to know a few healing spells or have some potions in his collection that he can share - for a price. Some of the more built up urban communities, such as Brightwall, have their own temples dedicated to healing and hospitals to care for the dying. The wealthy and the nobility can usually afford to hire their own private physicians, who have a more detailed knowledge of the healing arts than a priest or monk, while the higher nobility such as the Prince himself have their own full time court physicians.

The states of Western Pavonistade and Capital State has a universal healthcare system, bent on social health insurance as workers, enterprises, and the state governments work together to pay for the system. In all other 4 states, the healthcare system is privatized.

(I know this sounds quite unrealistic, but I'm trying to make my nation look decentralized and diverse.)

Who is responsible for public healthcare in Makaar?
Makaar operates a single-payer healthcare system, where the National Health Service owns all hospitals and employs all doctors and nurses. The NHS is responsible for implementing public health policy in Makaar and providing treatment to injured and ill people.

How is healthcare paid for?
Healthcare in Makaar is paid for through taxation. All healthcare is provided free at the point of need.

Who is entitled to medical care through the NHS?
Most people in Makaar are entitled to free comprehensive medical care - that is, they will receive free treatment from diagnosis through to the completion of a course of treatment aimed at eliminating their condition, or reducing its severity as far as possible. A person is entitled to comprehensive free medical care if they fall into one of the following categories:

  1. Makaarian citizens - for all medical conditions
  2. Holders of Permanent Residence Visas - for conditions which develop after issuance of the visa
  3. Children of holders of Permanent Residence Visas - for conditions which develop after issuance of the visa
  4. Holders of Temporary Work or Business Visas, if they have paid any tax whilst on that visa - for conditions which develop after issuance of the visa

Note that holders of PR, TW or TB visas are required to submit health questionnaires and, if judged necessary, to undergo a medical examination prior to entrance into Makaar. Conditions which were diagnosed before entrance into Makaar cannot be treated for free by the NHS and charges will have to be made.

Comprehensive healthcare is also available to those on tourist visas or without visas if they purchase government health insurance on arrival, or purchase private tourist health insurance before arrival. Private health insurance for tourists tends to be cheaper and as such is the most popular option, though government health insurance for tourists tends to be more comprehensive. Government health insurance applies only to conditions developed since arrival and not to pre-existing conditions; the terms of private health insurance vary.

Any person who does not fit into any of the above categories and does not hold any separate insurance policy is entitled to free emergency treatment. This means that they will be given all the treatment necessary to save their life or limb and to stabilise their condition to such an extent that they may travel back to their own country for long-term treatment.

Is there a private option?
No. All medicine, dentistry, pharmacology, nursing, and related medical courses in Makaarian colleges and universities are funded by the NHS, with the number of course places set each year based on the anticipated need for new workers. All medical workers are directly employed by the NHS following the completion of their studies. There are no gaps in the market for private practice to emerge, and few people willing to pay for it when they are already covered by the NHS.
Makaar is ranked 323rd in the world for Most Dedicated Public Healthcare.
Makaar is ranked 272nd in the world for Largest Welfare Programs (per capita).
Makaar is ranked 408th in the world for Nicest Citizens.

Healthcare in Costa Fiero is predominantly paid for using a three-payer social security system in which the employer, the employee and the government pay equal amounts of money into a health plan every two months. This plan is then used to cover surgeries and treatment at hospitals, both public and private. Additionally, all public hospitals and medical centers are owned and operated by the government and are free at the point of entry and for emergency treatment. All non-essential and all cosmetic treatment and procedures are however not covered by government expenses. The same applies to private hospitals and medical centers for emergency treatment only.

All citizens receive free healthcare (relatively, not including tax). Over the counter medicine does cost money though but it's cheap. Surgeries, vaccinations, and any emergency medical procedures won't cost the citizens a penny up front and there is no bill that will come back to them later.

There used to be universal healthcare via the single-payer Federal Health Service however in actuality the service is only available to 97% of Radiatians due to geographic isolation.

Furthermore, severe budget cuts have meant that the majority of citizens carry private health insurance, and it is predicted that the FHS will probably be scrapped altogether in the future.

I keep track of each of my Children's physical status practically from the moment they're conceived. Of course, I have to ration my time and resources taking care of them, but fortunately the state of my Children's medical technology is such that they're kept overall in very good health. Far better health than most of you xenos, actually. Most illnesses are preventable through proper diet, exercise, and hygiene, all of which I teach my Children from an early age.]

Healthcare in the The Socialist Union of Katayashastan is completely state controlled. So we do indeed have universal healthcare.

The Kingdom of Katayashastan on the other hand. Can't even afford a government building, and no business wants to set up there. So their only healthcare is some dude with smuggled drugs with no clue what they do, Not sure you can even call that healthcare........

In a society of cyborgs, universal healthcare kind of becomes a necessity.
Freedom doesn't mean being able to do as one please, but rather not to do as one doesn't please.

A fool sees religion as the truth. A smart man sees religion as a lie. A ruler sees religion as a useful tool.
