I will usually insert it very slowly, cautiously, and tactically... and only when I'm sure I have the position fairly narrowed down (through feeling and also how it all sounds in the ear), do I start the extraction phase. I'm very patient and usually I understand that this could be a 10-20 minute+ operation. It depends on the exact earwax mass and also how it's positioned.
