As best I can remember, the plan for the territory obtained in Brest-Litovsk was for it to be broken up into Bantustans ruled by German princelings.

Keeping the pieces it had had since the partition, yes.

But the pieces it took from Russia were to be turned into a Polish puppet state.

Obviously it they would not restore it to 1600s size, and the exact details uncertain, but it was the plan.
There would be one but the final borders not fully determined.
It is possible it would be united with the AH Kingdom of Galicia and Lodomeria to make a Austro Hungary Poland.

Russian Poland was going to be made its own Kingdom (in fact, the German army in Poland posed as their liberators, kind of ironic considering what would happen later on in the century).

If I recall correctly, they didn't mind creating Poland out of the Russian partition, though I don't think they had thought far enough to think "If we give Russian Poland independence, German Poles might want independence too"
