I would say it is a skill, and technically a speech impediment. An "accent" in sign language would be ASL versus old Native American sign language, versus signs for things in Britain or Canada. With entirely different "words"/terms or similar ones with vastly different meanings.

Granted, I've never done sign language in my life, but that's my two cents.
