Grounds for divorce/annulment are required. Given the status of women in Ephyra, it is usually requested by the man or the woman's family. A man can request an annulment if his wife is committing adultery for example, but doesn't have to. A woman's family can request an annulment if they believe he is treating her more like a slave than a wife, as they would regard this as an insult to their whole family.

In some more extreme cases, usually when a citizen family is raised to the position of lesser noble, or in just any other scenario where either a suitable male or female is lacking, it can occur that an uncle will marry a niece. In some cases, these can be similar aged, but the majority of the time the uncle is substantially older than the niece. These marriages are arranged, and rarely do either the male or the female have a say in this arrangement. The Anogaryen dynasty is the most fanatic of these, for as the Imperial family, they regard very few other bloodlines as being suitable to marry into.

That depends. A man will often justify not having sex with his wife and having sex with other women as seeking out concubines or even another wife. The most humiliating punishment a man can inflict on his wife for infidelity is the implementation of a chastity belt, to physically ensure she cannot have sex with another man, nor satisfy her lusts with anyone but her husband.
