I mean, while Phenyzian nuclear subs can technically launch their missiles on their own it still takes the commander of the submarine, the second in command and the political officer on board all three authorizing the launch and providing their part of the launch codes. Making it impossible for one rogue commander to do what happened in ZN.

Gonswanzan nuke subs cannot launch without direct orders from the president, if anyone even attempts to reach for the keys which are in a secure lockbox, they'd be executed on the spot. Even then, there's four keys that need to be turned by randomly assigned crew who change every time they are deployed, when the order is confirmed by the navigation officer, these four crew will get the confirmation code via the intercom, prior to moving for the lockbox which is opened by all four at once. Then they need to move for the launch control panels and synchronize the key turning to enable the system before it can finally be launched by the weapons officer.

A complex dance, just so some idiot can't say "YES, I'D LIKE TO NUKE SOME RANDOM CITY FOR THE HELL OF IT" and then go through with it.

Yea I ain't even bothering with that clusterfuck of a war that exploded from what was meant to be a minor "mafia" raid. Somehow. It just doesn't make any sense no matter how hard you twist it, I'm not even going to bother with intervening given it's already far beyond the point of no return. If you want something smaller scale, I do have a puppet I could use to set up something akin to a raid without jumping to a full-on war situation. After you wrap up your current conflict of course.

If comms are lost for whatever reason, despite backups, it would be safely assumed that Gonswanza is "dead" prior to launch within the hour (after it's confirmed that it's not on their own end). The type 3 carriers (technically just oversized missile carriers if you think about it hard enough) are more direct, though none of the captains in charge are delusional enough to go rogue. Simply just call up for launch, then do so. Captains have final say in orders, given the streamlined chain of command, but they CANNOT launch on a whim even if they "feel like it". President still needs to make the call unless contact is lost permanently with a grace period and confirmation that it's not on their own end.

Missile launch complexes however would be alerted via early warning systems, so even if comms are lost suddenly, it's more likely they'd get a massive hint of "oh, yes, we are under attack" before scrambling to get the alert out to everyone else with nukes, then launching in response once the threat is confirmed.

Only a few times did idiots almost end up with nukes thrown back due to suspect actions prior hinting at something else entirely, like with Muqaddasia throwing (non nuclear missiles) around despite a lack of actual reasoning, with confirmation from allies that they were, in fact, non nuclear. Though honestly that was more akin to using a fucking ICBM to send a receipt from Wal-Mart.

More or less yes. On a time delay*, however, with manual override as needed.

*As in, some time to CONFIRM that yes, Gonswanza was somehow wiped off the map... Instead of some stupid technical issue that could put the world at stake.

Though, given that Kotun is literally under Gonswanza, so deep that (most, if not all) earth-penetrating munitions can't really reach it, it's very unlikely that Kotun would just spontaneously cease to exist.
