Food / healthcare / midwifing helps babies not get sick // die, but does not determine the chance of pregnancy.

Hhm, I need more workers. I need to wait about 5-7 more years before my kids can become workers. In the meantime, my scholars will be figuring stuff out. Plus, I need more kids.

You know kids need to eat right?

You’re just not thinking in economies of scale.

Children need the same amount of food until they are ten, so obviously they aren’t growing - they merely are birthed full size, then undergo a massive growth spurt at age 10. Thus, starved children can be a food source for the real keepers of the broods. Given you need to eat about 9x your caloric mass every year, once you hit ten breeding couples, children are self-sustaining.
