It was the leg and not deep because the bullet was likely a 9 or 8.5 mm and Anna was wearing minimal armor. And by taking off the gauze she did release blood but the gauze was quickly back on. Finally, since Kyraina skipped ahead, it's fair to say that the hospital did clean her up. And I was offering to treat the wounded Marshals, not Wolfram.

It doesn't matter where or what size bullet, pistol rounds are bigger than rifles rounds, so you did more damage pulling it out. That's how that kind of thing works, not to mention the fact that the bullet put inherent pressure on the surrounding blood vessels, meaning that blood clots are common. Gauze can only soak up so much blood before it's useless anyway.
Not to mention the pain of getting shot and the pain of muscle contracting and tearing around a jagged piece of metal.
And you offered to treat the people who just shot and killed friendlies, over the actual friendlies?
