I believe it's the second one; but I'm not sure as I don't think we've ever had this question come up. If Camicon (the IC OP) says anything different, go with what he says.

Basically, everyone is infected already. If you die without your brain being destroyed, you will reanimate, regardless of how you actually died. Walker bites and scratches are so dangerous because they are harbouring a shit ton of bacteria and viruses, which antibiotics/bacterials are largely ineffective against (if you even had access to them). So if you get bit or scratched in an area that can be immediately amputated, you're screwed. Even amputation is no sure-fire thing, carrying it's own host of problems, with no guarantee that the deadly pathogens haven't already begun to spread through you body.

Not really, no. I'm working my way through the previous IC threads to create plot summaries, and get a comprehensive list of all past and present characters, but we don't have a list of the players behind those characters.
