Rhaegar and Lyanna were in the "protective custody" of the Lord Commander, Gerold Hightower. When the royalist armies were defeated a couple times, either Aerys decided he needed Rhaegar as a rallying symbol or Rhaegar persuaded his father to allow him to take the field. Lyanna was kept in the Tower of Joy at Dorne to ensure Rhaegar didn't turn around with his 40,000 men, capture Aerys, and try to end the war somehow else. Think about it, why would Rhaegar take Lyanna to Dorne? The Dornish hated her for replacing Princess Elia.

But it is also stated that the King (or Regent) can assign a Kingsguard to whoever they wish to, ex. Arys going with Myrcella.
