It's a slur for a person with bipolar disorder or schizophrenia. Mental illness of any kind is shameful in some cultures. It's
purposely heightened in some manga to paint a mentally ill person as evil or vile. They're made to feel ashamed rather than understood in society. The term is like a flash fire in American culture and anime fans run with it without realizing the harshness (and insult) behind it.
For example, let's substitute words...
"She's yandere," says Bob as he points to the girl with bipolar disorder.
"He's a nigger," says Sue as she points to the black guy.
"He's a fag," says John as he points to the homosexual boy.
"She's a retard," says Steve as he points to the kid with Asperger syndrome.
Those words are
very insulting to the people who are gay, or have AS, or are African-American, or suffer from a mental illness.
