Depends on the specific criteria. I'm sure there are standards.

It is worth noting that the Medal of Honor has not yet been awarded to anybody.

We have that too, its called the Corona Civica (civic crown), its awarded to soldiers who saved a civilian citizen by slaying an enemy on the spot

But the highes honor is the Corona obsidionalis, It is given only to generals whose actions saved legion from utter destruction or defeat.

In Ghanara, the highest honor is the Industrial Paragon award. It is a civilian award presented to the executive of the company that provides more economic activity than any other in Ghanara. Only one can hold this award at a time. Ghanara strives to attain the highest level of industrialism possible, and those that contribute economic growth to the nation are respected above all of the generals and diplomats. In addition to recieving this award, the recipient's business has the government invest a substantial sum of simoleons into it.

In Tigellinus, we have the following:

1) The Emperor's Will - For "repeated and admirable devotion and heroic action in the service of the Imperial Throne and the Citizens of the Imperium of Tigellinus at risk of life, limb, or great personal expense". The Imperium's highest award; constitutes both civil and military service; either Imperial Medallion of Service or Medal of Distinguished Imperial Honor must be acquired first.

2.1) Medal of Distinguished Imperial Honor - For "conspicuous, noteworthy, and heroic service and sacrifice during action against an enemy of the Imperium of Tigellinus". Highest military award.

2.2) Imperial Medallion of Service - For " exceptional, noteworthy, and commendable civil service, at great expense, to the Imperial Throne and the Citizens of the Imperium of Tigellinus." Highest civilian award.

3) Laureate of the Imperium - For "the contribution of significant cultural and scientific advances to the Citizens of the Imperium of Tigellinus." Much like the Nobel Prize IRL; contains Physics, Chemistry, Physiology /Medicine, and Literature categories; foreign nominees are also allowed if contribution was especially significant.

The highest medal of honour for the armed forces - all get the same - is the George Cross, and is awarded for bravery in the face of enemy fire:

Image

For either civilians that show bravery, or armed forces that show bravery but not in the face of fire, the Victoria Cross is awarded:

A person who gets either award is allowed to show either GC or VC after their name.

Militarily: "The Medal of Honour". As such, only 9 people in the history of the Empire holds this honour, among them the previous Commander of the Armed Forces.

Civilian: "The Medal of Righteousness". As such, about 498 people have received this honour, due to every Emperor before the Restoration being granted one. After the Restoration, only 3 people received this honour; the Emperor Meiji Vankolk, the public spokesman and former Leader of the Socialist Party Sixton Lok, and President Aragon Brisingr, the current President of the Keronian Empire.

The highest Radictistani honor is the Radictistani Cross, awarded "For the display of unnautural in the face of the enemy beyond the normal requirements of duty, at great risk of life and limb." The recipient is entitled to the postnomial letters"RC."

The highest civil honor, not including peerages, is to be made a Knight Grand Cross of The Most Noble Order of The Grand Duchy of Radictistan. This entitles the recipient to a knightly title and the postnomial letters "GCR."

The Highest honor is only presented to the Dragoons of Ruskarkand, and only for one reason:
For showing your honor to your species, and doing your part for the nation (A difficult task indeed, only 17 Dragoons have been able to get such an award, and those said Dragoons are revered amongst the nation, they are celebrated)

The highest honor you can earn in Alevuss is available to both soldiers, citizens, government officials, immigrants, and foreigners. It is known as La Médaille de la Mettle Souvenir éternel. It is earned through the means of "protection and preservation of the livelihood of Alevuss, a peaceful entity/organization/movement, and/or Earth and/or the betterment of society."

The honor comes in the form of a medal attacked to a ribbon to be placed around the owner's/owners' throat or however they wish to wear it. The medals used for its construction have varied throughout the ages of Alevuss, but it has always come with a cash prize, which has also varied by the ages, and the promise to defend the individual's/individuals' well-being as long as they are still seen as deserving of the medal.

The La Médaille de la Mettle Souvenir éternel has rarely been handed out, but a few well-known Alevian possessors of it include Joseph Calvin, Jacob Casey, and Jessica Roonlee.

There are three different high-ranking medals:

First, there is the Saurian Medal of Excellent Service, awarded towards Dinosaurs, Marine Reptiles, and Pterosaurs who have performed outstandingly on the battlefield.

Second, there is the Golden Raptor, awarded towards individuals who have performed heroically on the battlefield and have come out alive in the end.

Finally, there is the Medal of Eternal Valor, awarded only towards those who have fought bravely and were either injured or have been killed in action.

Then, there are two titles: Hero of Saurisia, awarded to those who have gone above and beyond the call of duty to serve Saurisia. And then, there's Defender of Dinosaurs, awarded ONLY towards those Dinosaurs who have sacrificed everything to protect their kind from the evils of Human Supremacy.
