Oii, DO,
The first thing coming into my mind would be the Despotate, which you've already included into your note-to-self. It's certainly a thing to include, as a Despotes (or Despoina, if female) was a remarkably frequent title for a ruler nominally dependent on the Emperor of the Byzantine Empire in the 12th and 13th century. It was also a title rulers of medieval Serbia used in the 14th and 15 century in the Byzantine style, and in Hungary and the countries which form Romania today, this title was used for a while as well.
I would also keep the types 'Federal Republic'/'Confederated Republic' and 'Federation'/'Confederation'/'Confederacy'/etc. separately, as a federation or confederation does not necessarily need to be a republic or consist of republics. It can have a republican federal constitution and consist of monarchies (like the UAE), it can even consist of both republics and monarchies (like Malaysia), it can even be a monarchy consisting of monarchies and republics alike (like historically the HRE, the North German Confederation, the German Empire etc. - in all these iterations, there were kingdoms like Prussia and lots of duchies, principalities etc. included, but also city-states with republican constitutions, like the Hanseatic Cities of Bremen, Hamburg, and LĂĽbeck). Needless to mention, that the leader's title may vary a lot depending on the kind of formation and the geographic/cultural area these federations and confederacies were historically formed in.
For a republic, one could also add Free State/Freistaat into this, a synonym for a republic in a country inspired by Germany. It can be an independent country or a state of a federation/confederation. The German states of Bavaria, Saxony, and Thuringia use this pretitle to this day; like in all German states except for Berlin, Bremen, and Hamburg, which are city-states, the title is Ministerpräsident (Prime Minister or Governor).
I would also like to add 'Shogunate' and 'Daimyate' into the pool, with Shogun being the title of a military ruler from warrior nobility in a country inspired by feudal (pre-Meiji) Japan, and Daimyo being the title of a local ruler of a han ('fiefdom' or 'domain') who is part of the Shogun's tributary system.
That's all that comes into my mind so far. I'll get back to this once I get to new ideas. ^^

Greetings,
Lillo
