The LGBT+ Rights Survey is a method of avaliating nations human rights, equality and opportunity related to the LGBT+ population. The Survey uses the same parameters as the Rainbow Europe LGBT+ Rights Survey (click on it to go to Rainbow Europe website). However, there are none active LGBT+ Rights Survey currently, so the government of The Norden Utopia decided to remake the questionnaire, adding new questions about more current topics. Each question has a value attached to it. The index varies from 0 (blatant LGBT+ human rights violations) to 10 (full protection of LGBT+ human rights). There are 6 categories used to measure the final index, and they are: Equality & Non-discrimination, Family, Hate Crime and Hate Speech, Legal Gender Recognition and Bodily Integrity, Civil Society Space and Asylum

Classifications

Very High(score 8.5 - 10): Nations ranked in this category assign full protection of LGBT+ human rights, equality and opportunity. The government, and the populace, is truly committed to protect the rights of the LGBT+ people, problems, if any, are minimal.
High(score 7 - 8.5): Nations ranked in this category assign very high protection of LGBT+ human rights, equality and opportunity. The government is committed to protecting LGBT+ rights, however, there are noticeable flaws, but reform is near.
Moderate-High(score 6 - 7): Nations ranked in this category assign partial protection of LGBT+ human rights, equality and opportunity. The government is, up to some point, committed to protect LGBT+ rights, however, there are major flaws, but reform is not that far away.
Moderate(score 4 - 6): Nations ranked in this category assign minimal protection of LGBT+ human rights, equality and opportunity. The government does not seem to care enough for its LGBT+ populace, as a result there are noticeable problems for the LGBT+ population. Society is progressing however, and reform, albeit far, is possible.

Just realized that in South Aurora's replies there is a particular one very ambiguous and easy to misunderstand, so for further clarification:
Are transgender parents recognised on birth certificates, documents, etc...? Just as cis parents.

Is intended to mean:
Are transgender parents recognised on birth certificates, documents, etc...? Exactly like cis parents; furthermore their preferred gender is recognized.

The Nikopolian Empire and the Archoncy of Thama exists on the planet Asnea in the year 1849 PA (roughly 5431 CE/AD on Earth).
Human culture on the planet developed entirely out of the 22nd century Earth human scientists, engineers, skilled trades- and craftspeople, and soldiers who colonised the planet. This unique starting population of academics has given rise, over the 2989 years of colonisation, to languages which have no concept of gender, to cultures of equity, and to virtual incomprehensibility of the concepts of discriminating against people for their sex, gender, or their romantic and sexual orientations.
This may leave the impression that many Nikopolian and Asnean laws are lacking in specific protections, but this is not malice or ignorance or incompetence: there is simply no reason to legislate to protect people from something that literally doesn't happen.

No human government on Asnea has a legal concept of sex, gender, or orientation. To the government, people are people, their gender is irrelevant, their sex is irrelevant, and their sexual and romantic preferences are irrelevant. There is no legal distinction between cis and trans people nor between pan, gay, or straight people because both the cultures and languages of Asnean humans do not discriminate between people based on any of these concepts. In 21st century terms, the entire human population of the planet can be considered de jure Pansexual and Agender, and in practice this is also usually the case.

Sex is moderately different, and it is treated like other variable characteristics such as height, eye colour, or the predisposition of freckling versus tanning - people are sorted into the XYZ/O system based on their sexual characteristics. X is for people born with vaginas, Y for those with penises, Z for people with both (not considered intersex, true hermaphroditic people now account for roughly 2/5 of all human births), and ZO for people with divergent sexual characteristics other than those main three (intersex people). This data is stored and used only in their medical records, which are accessible only by them and authorized persons like doctors and parents. Parents of ZO infants are allowed to choose to start their child on Gene therapy to conform them to the Z category so long as the child is under the age of 2, but this is generally discouraged in favour of letting the child decide once they start puberty. The changing of one's physical sex is treated with the same social urgency as getting a nosejob, and there is no social distinction between people of different sexes.
