Healthcare in Your Nation

How does healthcare work in your nation? Is it a fully private system? Is it single-payer universal? Is it a two-tier system, combining the former two? Is it something else entirely? A sort of Frankenstein's monster hybrid of the former two?




The New Earth Republic maintains a single-payer universal health care system modeled on the National Health Service of the United Kingdom. Prescription drugs are provided but a dispensing fee must still be payed. Private practices are allowed and citizens may go to them for faster-but-more expensive treatment rather than rely on the Republic Health Service, which may take 6 weeks or more for treatment. The Republic Health System is paid for by the federal government, but individual territorial governments administrate it locally. It is paid for by general federal spending, encompassing about 16% of annual federal spending and 8% of the Republic's gross domestic product.

The Republic Health System is managed by the Health Directorate, which also controls the approval and patenting of drugs and negotiates prices for the RHS. The Health Directorate also shares authority over the Republic Emergency Management Agency alongside the Infrastructure Directorate and Policing Directorate. Finally, the Health Directorate also institutes the eugenics program of the New Earth Republic and is in charge of regulating human augmentation technology. The semi-autonomous Augmentation Authority exists for this purpose and several proposals to make an independent Augmentation Directorate have been considered, though they were rejected.

Of the four major political fronts, only the Liberal Front truly wishes to change this system (in favor of a more privatization). Both the Progressive and Conservative Fronts are both supportive of the status quo, while the Populists wish to take the universal healthcare idea even further and outlaw all private medical care. As the Health Directorate itself is overwhelmingly in favor of single-payer universal healthcare, even the Liberals are rarely willing to push against them and actually try to implement private healthcare; it's more a political statement than a legitimate policy goal.

Esperanzia has a universal, tax-funded single-payer system called the People's Health Service (Servicio Popiular de Salud). It was founded in 1960 and it replaced a previous system which was funded by mandatory health insurance (like Obamacare) and it was modeled on the UK's NHS. It provides all areas of healthcare free at the point of use (including dental and optical services and prescriptions) and is not means tested. Private healthcare (apart from cosmetic surgery) is illegal.

Arkiasis has a publicly funded healthcare system administered on a federal level by the Arkiasis Health Service (AHS) which guarantees comprehensive health insurance to all citizens and permanent residents. The AHS was enacted in 1948 by the Parliament of Arkiasis and signed into law by President Andrew Bradley making it the 2nd oldest (after the Atlantican Health Service) and largest single-payer healthcare system in the world. Private Healthcare is legally limited to primarily cosmetic surgeries with the AHS being the sole Health Insurance provider in Arkiasis. Health care in Arkiasis in funded through general taxation and is available free at the point of use (above a certain level) with patients freely being able to select the physicians or facilities of their choice.

Arkiasis ranks highly in general health and has one of the lowest rates of obesity of any developed nation in the world. Arkiasis has a life expectancy of 82 years and an infant mortality rate of 3.63/1000 births. There are 3.16 physicians per 1000 people and 5.9 hospital beds per 1000 people. Health care constitutes 10.2% of the national GDP or $4,204.37 per capita. Healthcare costs have begun to steadily increase as the population of Arkiasis continues to age. According to a survey in 2009 despite liberal drug laws very few Arkiasians are drug users with only 7% of Arkiasians consuming tobacco products. The suicide rate in Arkiasis was 11.6/100,000 in 2011, an increase from a rate of 10.3 in 1990.

Healthcare is universally available here.

It is not payed for in any sense, either by private citizens or through taxation, as Victor does not use a monetary system of any kind. No currency changes hands when a citizen requires medical attention. We simply treat people when they need it.

There are instances where an individual does not receive treatment when they need it. Such instances do not include an inability to pay, but do include when
-the individual actively refuses treatment, or has preemptively refused it prior to their injury / illness
-the treatment would be exorbitantly costly when compared to the value of one citizen's life (such as requiring the sacrifice of two other healthy citizens)
-the necessary procedures are unknown to current science or otherwise unavailable

It's universal healthcare, given it's the third most important priority in the budget, after Education and Social Welfare. Although there are private hospitals (after all, Verdiga IS capitalist.)

Healthcare in Gandoor is solely provided by the Gandoor National Health Insurance, a publicly-funded health insurance available to all citizens and permanent residents in Gandoor for all purposes, barring cosmetic surgery. In addition, foreigners are covered in the event of medical emergencies. The GNHI was established in 1961 and private health insurance was phased out by 1973.

Ostensibly Radiatia has universal health care via the Federal Health Service (FHS) which is free at the point of use. Originally the FHS was funded and administered by the federal government, but it has since fallen to state governments to administer the system.

As such, the FHS is grossly underfunded in most states and most Radiatians therefore rely on private health insurance and private health providers.

Healthcare in New Phyrreus is handled on a nationwide level by the Phyrrean Department of Health (PDH), and locally by individual district conclaves. New Phyrreus is often considered one of the world's leaders in medical technologies, with significant influence in the fields of gene therapy, nano-surgery, oncology, neuroscience and reconstructive surgery.

This. There are also "concierge doctors", primarily in wealthy communities like Pretty Lake, West Olwe, not unlike Hankmed on Royal Pains.

It should also be mentioned that all healthcare providers -- even non-government ones -- are subject to the Keep Olweans Employed Act, which imposes heavy tax penalties on companies that don't have a real person answering their phones, keep their bank accounts outside Olwe and/or outsource jobs to other countries.

In Aikoland, healthcare is entirely publicly funded and all hospitals and other medical facilities are ran by the government and all employees are officially classified as government workers. Unfortunately, usage of our healthcare service is limited to citizens and permanent legal residents, foreigners will either have to pay out of pocket for treatment (it's still fairly unexpensive) or be covered by their own insurance, as our hospitals will accept the vast majority of foreign health insurance plans as well.

The Royal Health Service was introduced in 2010 and is a universal health care system. Under this umbrella includes the free provision of basic medical procedures and services, certain social care and support and certain educational programs for general health and well being.

A healthy population is essential, not only for its own sake but also to provide an efficient workforce for a stable economy. Therefore, all healthcare in Fatatatutti is taxpayer-funded and free of charge to the end-user.

Note: There are some elective medical procedures, for example cosmetic surgery, which are not government-funded but can be obtained on a pay-as-you-go basis. Those procedures are not considered "healthcare" per se.

Healthcare in Apiatica is primitive and low-tech; care is geared primarily towards keeping pupae alive and seeing them through to birth, not really care what happens after that. All care for the brood is provided through the hive via taxes, as well as a few doctors who double as both brood facilitators and as regular practicioners. They usually charge $1-$2 a visit, which makes them quite rich by apian standards. Unfortunately, most patients die from their ailments regardless.

The Republic of the Greater Cascades has a single-payer public option called the Universal Healthcare Initiative, or UHI ("YOO-hi"), administrated by the Department of Social and Health Services. The Universal Health Initiative was modeled after H.R. 676, a bill first introduced to the 112th United States Congress. H.R. 676 would create a "Medicare For All" system that would expand the provisions of Medicare and Medicaid to cover all Americans.

UHI creates a publicly financed, privately delivered health care system that improves and expands the already existing Medicare/Medicaid programs to all residents of Cascadia. All Cascadians have access, guaranteed by law, to the highest quality and most cost effective health care services regardless of their employment, income or health care status. In short, health care becomes a fundamental human right, with no financial barriers or financial harm resulting from seeking care. The Universal Healthcare Initiative uses the single-payer model to:

• Reduce national expenditures on healthcare by streamlining billing and administrative processes;
• Increase health insurance enrollment and coverage rates;
• Creation of an all-payer rate system to bring private insurers to standardized rates for medical procedures;
• Using all-payer rate systems to transition private insurers to public options within ten years to reduce the possibility of insurance plan cancellations;
• Standardize healthcare administrative and billing protocols under the umbrella of a public agency.

This program covers all medically necessary services, including primary care, medically approved diet and nutrition services, inpatient care, outpatient care, emergency care, prescription drugs, durable medical equipment, hearing services, long term care, palliative care, podiatric care, mental health services, dentistry, oral surgery, eye care, chiropractic, and substance abuse treatment. Patients have their choice of physicians, providers, hospitals, clinics, and practices. There are no co-pays or deductibles allowed in UHI.

The Universal Healthcare Initiative will negotiate reimbursement rates annually with physicians, allow for global budgets (monthly lump sums for operating expenses) for hospitals, and negotiate prices for prescription drugs, medical supplies and equipment. The Universal Health Operating Trust Fund ensures a dedicated stream of funding. An annual legislative appropriation is also authorized to ensure optimal levels of funding for the program, in particular, to ensure the requisite number of physicians and nurses need in the health care delivery system.

Funding the Universal Health Operating Trust Fund initially maintains current national funding for healthcare, raises personal income taxes on the top 5% of income earners, establishes a progressive excise tax on payroll on self-employement income, establishes a tax on unearned income (namely capital gains and real estate property valuations), and establishes a tax on bond transactions.
