In seocol, yes, the government even allows them to change there identity as log they look like women/men. The government pays for about 15% of the surgery cost.

"It is legal in theory. Any Dizyntk may undergo any cosmetic procedure they wish as long as they have the funds for the operation and can find a healer with the necessary expertise. We have never put it into practice however as I have never heard of a Dizyntk wanting such an odd thing. Furthermore I am not sure that any of the medical training facilities offer instruction in such a strange procedure. One might have difficulty finding a healer to preform the operation even if it was desired."

It's legal, but you must sustain the financial costs entirely yourself, and then you must deal with the lingering social stigma.

It does seem a touch on the extreme side, doesn't it?

Most people who get sex changes in the Federal Republic have spent years preparing, saving the necessary money to handle the costs, and making any preparations they feel are necessary. Most will legally change their names once the deed is done as well, and relocate to a different part of the country. This is a holdover, from back in the days when the stigma held against such things was much worse.

Because there is no National Healthcare System, and because no major insurance providers will sustain the costs of gender reassignment, sex changes are still a bit rare compared to many other countries.

To obtain a sex change one must first get the approval of at least two doctors, in whose professional opinion remaining the sex you were born as would be severely psychologically traumatic. Otherwise, if you want it, you'll have to pay for it yourself (and it's not exactly cheap). Among the Dvardi'i the procedure is viewed as slightly odd, but no more so than any other form of cosmetic surgery. They typically have more important things to worry about, anyway.

(Worth clarifying -- state-run healthcare covers only emergency care, and only for those without health insurance. Those with health insurance will find that almost all providers will only cover surgery if it's deemed medically necessary, as above.)

OOC: And to keep people from propagating a rather annoyingly patronising myth... transsexuality has nothing to do with socialisation or gender roles. It's caused by what's basically a body dysmorphic disorder -- the brain is "typed" as being the opposite sex from the physical body. As such, transsexuals tend to realise they are different at a very early age. It is not genetic (and as such cannot be "bred out" through genetic engineering), it cannot be trained out (any more than a blue-eyed person can be "trained" to have brown eyes) and apart from changing the sex of the affected individual the only theoretical "cure" is some advanced FT-level brain surgery performed at a sufficiently early age as to not harm the individual psychologically. As well, every human society, however "enlightened", will still have a transsexual population. They might go into hiding if it's not socially acceptable, but much like homosexuals they can't be stamped out.

Yes, it's almost never done though. There have been about 15 such operations performed in Southern Babylonia's history. It is not, however, considered a part of healthcare. The patients will normally have to pay for it themselves, but private charities will often donate them some money for such things (as the government rewards companies who do that stuff a lot with a minor tax cut).
