Well, if a person strongly feels that they are a particular gender, then without evidence to the contrary, I have to believe that they believe they are that gender, given how common of an issue this is, it makes sense to treat gender as both a mental and physical state.

When people started treating me as female, it made quite a difference to me. I felt more like myself, if that makes sense. So a little common courtesy of treating someone as the gender they are goes a long way.

Well, if you identify yourself as female, then I identify you as female. For me it's not really a matter of conscious effort, but automatic acceptance. This is how you wish to present yourself, and therefore this is how you are percieved.

Well, sex and gender are different concepts. Having two standard human biological sexes (and a few non-standard ones) is useful and necessary, especially from a medical perspective. Obviously penis isn't the sole indicator of being of the male sex, but it is one of the more... eye-catching ones.
