It's used as a verbal slap in the face intended to demonstrate how superior the speaker is to the recipient, while pretending that it's meant as a kindness and still allowing them to feel good about themselves. And everyone who says it always has exactly the same tone, too. A facade of friendly concern masking sneering condescension.
