I was wondering this too, it feels like only a few years ago he gambled wrong and lost the premiership? (Am I making that up?) Having not followed Italian politics especially closely since then, how the hell did he pull off this comeback?

Well, he was still a Deputy, and he basically just collected some of his followers together and broke off from the PD to form Italia Viva.

He was secretary of PD when the candidates for the 2018 elections were decided. So he got to appoint the PD candidates.
He took for himself the Senate seat for his own hometown Rignano, and placed his most trusted people in "safe" seats. Then he took them with himself when he left the PD to form Italia Viva.
