Possible that he is simply in lockdown from the virus. Maybe it hit his country and is wreaking havoc.

Possible that he contracted rona and is really sick from it (smoker + fat).

Possible he died

North Korea is extremely conservative so it is unlikely that his sister will be able to take control even if she is the most capable candidate. His brother is reportedly apolitical and simply plays music. His children are too young to grab power for themselves. This leaves Kim Pyong Il or a general.
Whoever said "you can lead a horse to water but you can't make them drink" has clearly never drown a horse.
