Unironically, though, a lot of North Korea's myth as a state hedges on them existing for the well-being of the worker and citizen, to achieve glorious Marxist ideals for all. If this narrative were broken and the Kims were outright declared emperor, they would lose a lot of image they've spent decades building up. Such a shock to the national psyche could be disastrous.

A national mythos is a powerful tool, and the way North Korea projects itself domestically is no accident.

Personally I'd fully believe he does have some serious health issues. He's heavily obese and by a lot of reports does lots of unhealthy things, on top of the stress from running a nation. But I don't think he's dead or braindead, ROK intelligence would be able to pick up on that fast as there would be powerplays like none other in the event of a sudden Kim death.
