As a fairly avid North Korea watcher I'll echo some sentiments here and say you should take everything regarding North Korea with a massive grain of salt, no matter how many "anonymous officials" say it to news agencies. A huge amount of things said by such people have been later proven to be false.

My favorite examples are the stories of someone being executed in a bizarre and extreme fashion only to later show up in a photograph or video background later down the line.

While unlikely there's enough stress points to tip things over given the right, or wrong I guess, nudge. The massive economic impact will have ramifications, perhaps not end of humanity ramifications certainly, but then there's a high potential for great change.

How does Kim Jong Un have problems at such a relatively young age? Does he have a ton of body fat or have poor nutrition/lifestyle? I don't work out religiously and live the lazy and comfortable life. And even I haven't deteriorated much. Its usually sufficient to eat well in picking the right foods, maybe fast for long enough each day, get your sleep, and not drink alcohol or take bad risks.

Seriously though, everyone’s blowing things out of proportion, the only thing that will make history textbooks from this year is probably coronavirus and the US election, and maybe a war between Egypt and Ethiopia. Not WW2 or the end of humanity or whatever edgy teens are saying

I'd fully believe he has some sort of health issues given the things we know about his behavior and his physical health, but I'd be deeply skeptical of claims that he's braindead or whatever.

There were reports a few years ago during the "rocket man" missile scare that he had a heart attack, but those were false, so I have a feeling this is wrong intelligence

He is obese, but still relatively young. That and he would have access to decent healthcare, even if few people in his country do. Unless he suffered a stroke or a cardiac event as this young of an age (which, not super likely), I can't speculate as to what it might be that potentially incapacitated him like this. Doubt it's COVID, seeing as few people travel into N.K, I do believe that there are very few cases in that country.

Either way, if he kicks it (which doesn't seem super likely), the lack of a viable successor could cause more problems than one might think (as the OP did mention), but simultaneously, Jong-Un is a piece of dirt, yada yada, won't beat a dead horse, so I don't think many would shed a tear about his demise, but the repercussions could harm the North Korean people even more.

Actually due to his obesity I would say that a heart attack is very likely. Those who are obese are at risk for high blood pressure and high cholesterol bother of which raise the risk of a heart attack significantly

From what I know, it's widely speculated that Kim Yo-jong, Kim's sister, would be the one to succeed him. This is primarily due to the fact that she's increasing her roles right now and previously did so during his last health scare.
