It depends. Fetish pornography usage is a risk factor towards poor sexual functionality, but drink and drugs are liable to damage the viability of more immediately crucial organs. On the other hand, I imagine the former is rather more embarrassing to talk to a medical professional about in the event of a destructive addiction. That poor doctor.
