Well that's a specialist. They've got labs with expensive equipment and specialized chemicals your doctor could never afford. Or if they did, your doctor would cost you more every time you go in for a checkup, to help pay for all that stuff you rarely need. It simply makes no sense to push back against the specialization of medical services, you might as well complain that your car isn't getting the head gasket it needs when you pull in to fill it up.
GP's are for regular maintenance AND referral to specialists. I trust my GP, but only to the limits of his knowledge. I would never accept "well it's probably just a heart murmur there's no fixing that". I expect him (my GP is male) to send me to a specialist for doppler and ECG. It's my heart, not a fucking ingrown toenail.

Monopolies don't require contractual renewal every set number of years.

The blood tests cost too much money, so I propose that by default- it shouldn't be done. Let people pay out of pocket for it, if they want a specialist service done. GPs shouldn't be for referral to specialists, let them go back to their primary role of maintenance and facilitating preventive health. It'll encourage people to make better lifestyle choices and disincentivize wasting tons of resources on keeping already unhealthy people alive.
People can still be unhealthy all they want, but they should pay out of pocket for that privilege and not expect the government to foot the bill for them. It'd save a ton of money to put lots of specialists and middle men out of a job. The specialists who are worth what they charge, can service the wealthy or those who have insurance.

Heath insurance fundamentally is different from this.
Everyone uses it, and for the people that really need it, chronic diseases, it’s predictable not random.
So in those cases hey are loosing money, while in the first you pay someone to pay you less back.
The only part that is economically viable is the random incident, 400$ bill to fix a broken bone.
And 50 years ago, most people didn't have Health insurance, not to because they couldn’t , it because they didn't need too. Nowadays drug prices are super inflated, that’s the issue. Take that away and you won’t need Insurance to pay for a flu shot, or antibiotics.
Usually competition will quickly drop prices To competitive rates, cuz, those business can survive with a couple less millions, there flooded with money right now...
