Typically, if your behaviour suggests you're a threat to yourself or others they can petition a court to get you committed. Trying to Mission Impossible your way out of the Hospital without going through proper channels would generally indicate you either have something to hide, or are probably crazy.
Normally to get out of a Hospital you HAVE to be discharged. Either because the doctors think you are ok, or because you've signed away your right to sue them because you're going against their medical advice. At which point the whole "So how are you gonna pay for this." Question comes up.

This doesn't answer the question of Americans who receive emergency medical care that cannot afford to pay for it, though. So, what are we to do, let their lives crumble before them because they happened to get a disease and can't afford the treatment for it?
Of course, a nation has a duty to its own citizens first, but illegal immigrants who need care should get it. The fact that they illegally immigrated to the U.S. is irrelevant when it's a life-or-death scenario. It's still morally contemptible to let them die simply because they can't pay for it.

Yes, if they're an illegal immigrant, and they have no money for healthcare, compensate the hospital as much you can with whatever property they have before deporting them.
