Custom pre-titles?

I see many people with pre-titles that aren't on the list when you make a country. How does one do this?

Get to a population of 500 million, then the list will become a custom field.
There are some people that are there purely to oppose everything I say or do.

And getting to that population takes approximately three months from a nation's creation.

Population = just under 20 million. Economy = only Thriving. Average Life expectancy = c.60 years. If the nation is classified as 'Anarchy' there still is a [strictly limited] national government... and those aren't "biker gangs", they're traditional cross-Clan 'Warrior Societies', generally respected rather than feared.
