Prosthetics exist.

They’re expensive and don’t work as well.
And it’s supposed possibly to wear away someone’s skull with a stone (although painful and susceptible to illnesses) with killing them. Ancients used this to ‘stop’ disease (it didn’t work, although most medicine has always been a scam) but it could work as punishment that stays with them lifelong.

I'd support having a monarchy with real power along with some kind of upper house representing the civil society. Apparently Slovenia has something like this.
