Well, the show has shown that regeneration can change sex of a Time Lord or Lady. I don't think it should raise questions that weren't asked when Missy showed up, or when 11 mentioned the Corsair.
That's the first time I started questioning how Gallifreyans even worked. Do they choose their new form or is it random? Does it even have to be human in appearance? Can the Doctor take the form of a Dalek or a Cyberman if he wanted?
I admit I assumed a few things due to the Doctor's relative consistency up until that point.
