If we are talking about anything anywhere remotely resembling Emperor of the United States, we could look at the late Roman Republic rather than european Monarchies. After all the Principate developed during the transformation of the Roman Republic to an Empire. The US have not (yet) undergone this point, though they came very close to it with Douglas MacArthur's War in Korea and his standoff with President Truman.

That does not rule out that in the future the US might be getting close to the Rubicon once again, under whatever circumstances.

A world where America was a German monarchy would be way different.

Would they side with the central powers in WW1 (if WW1 even happened close to how it was originally)?

Given how the monarchs of Britain, Russia (pretty much 99%), Greece and Romania, all of whom were part of the Allies, were all German, I doubt that it would have played much of a factor.
