You can page through the Wiki articles on the various conventions. I seem to recall that some South Carolina delegates at the RNC were disqualified because SC held their primary earlier than they were scheduled to by the Party. I'm sure there were some disputes about delegates in the past, too.

Some conventions in the past were not a foregone conclusion. Kennedy was nominated with a narrow majority in 1960. The DNC voting went three ballots in 1952. RFK's death in 1968 threw that DNC into a bit of turmoil.
