Personally I have trouble distinguishing between "kowai" (scary) and "kawaii" (cute) in anime.

There's a stress in the ''ee'' sound, which can help distinguish between ''onii-chan'' and ''onee-san''. Or at least that was my impression whe I took Japanese. Also, well, context.
