They'll just ask you whether or not your intention was to relinquish U.S. Citizenship when you decided to become a citizen of the UK/Australia/Canada. If you reply that you were not the officer will just note that it wasn't your intention and that for all purposes you still retain U.S. Citizenship.

If you reply yes they will tell you to fill a questionnaire to ascertain your intent to relinquish. After that they'll make you sign another paper which confirms you are voluntarily relinquishing U.S. Citizenship, which will then be sent and approved by the Department of State.

They don't generally ask you until you step into a consulate since the presumption that you want to retain U.S. Citizenship is established as a de facto thing, meaning that even if you get your UK/Aussie/Canadian citizenship the assumption is that no, you don't want to lose your U.S. citizenship.

When you go to a consular officer they are going to ask you though. You deal directly with the Consulate of the United States for your U.S. Citizenship things, whereas you deal with the UK/Aussie/Canadian Embassy/Consulate for your legal matters in those countries as an immigrant of those countries.

No no no, you can not have a dual American citizenship easily. It is certainly doable but Check with the state dept first, they will tell her.
