A trained medical professional would see genitals simply as part of the body under his/her care. A total stranger may not see it like that.

If nudity were socially acceptable, then one would be used to total strangers seeing one's genitals and they would be used to you seeing theirs. It would just be a fact of life - "yeah, humans have genitals... so what?"
