IMO, we're born without sexualities, and grow them as we develop, just like about everything else mental. Genderwise, same deal. As a newborn, a child is agendered, it doesn't care what the fuck it is, it's just a child, and can barely think for itself. Once it starts to learn about what gender is, it develops a position on it.
At least, that's how I see it.

At this point, it shouldn't even really be an issue for him for at least a couple of years yet. Most children don't begin puberty before age 9 or 10 at the very earliest. Any earlier than that and you're dealing with precocious puberty. But since the treatments won't cause any irreversible changes and it's far easier to transition without going through puberty as the wrong gender, I would support him and his parents if this is what they want to do.
