Makudo in Japan (that's the proper Osaka rendering of McDonalds) had some occassional good seasonal burgers, but their US "regional style ones were horrid.
MOS Burger (local J chain - yep, pronounced the same as "moss") had the good stuff - "rice burgers". They're made using pressed rice for a bun and the ingredients included wonderful stuff like yakiniku.
