They tried that. the town hall meeting was over and they were still screaming.

The screamers and chanters show up at the meetings because that's when the meetings are scheduled. If the planners arranged for an "Equal Time" general protest session first, they'd show up then and do their ranting and raving. Then, "Fair is fair; time for others that _aren't_ screaming to be heard." Move on to the actual, more traditional town hall meeting.
UNFORTUNATELY, I doubt this would work because too many of the loud crowd don't want for there to be any civil discourse. That might actually lead to a resolution and actual healthcare reform. Their actual objective is to simply shut down the process entirely. So even had their opportunity to express their views, they would most likely do everything they can to make sure that others do NOT get the same opportunity.
