For the republic, spread out a certain amount of land to vassals or in this case the elite, add a few sub-vassals for the slightly less elite. Elective succession would be implemented. The title of Consul would only control a select few provinces in Italy and a few loyalty regions from their commanding days. I mean Rome was a poorly protected Oligarchy a handful of families would dominate politics.
The problem is two Consuls and a system of fasces. Though typically a certain Consul would be more prominent and for CK2 a system of Prestige>Prestige for leading Consul would work.
The main problem is the lack of lineage tied to a major title. Basically it'd be very chaotic and i'm against Rome 2's every player leader is Dictator for life.
