You also need one of the Sayyid and Mirza traits, 1000 piety or a religious bloodline.
No, for Qarmatians you need 3 holy sites, Sayyid/Mirza, and piety or a bloodline. You need 500 piety to actually make it, you keep the 500 after title creation, and you access it via the religion menu by clicking on the CoA of the religious head. Furthermore, if you started in 936 you have the bloodline, so you don't need any additional piety, land, or ancestral relations. You only need specifically the piety to make the title.
