I've been wondering about this, the Avatar's replacement isn't born as soon as the old one dies, correct? Because I remember Roku was an adult when he became the Avatar.

Roku didn't "become" the Avatar, he just discovered he was the Avatar.

Right, the avatar dies and one is reincarnated. Then, a new one is born in the next nation. The baby selects toys which determines the avatar. They are not told they are the avatar until they are 16. Aang was told at 12 because the Fire Nation was becoming more militaristic.

Now, I am not sure if the reincarnation process starts when the avatar dies or before they die.

I think the toy thing was just specific to Aang. If I recall correctly, a bunch of monks told Roku he was the Avatar.

Yeah, the episode from the first series all about the history of the war and the falling out between Roku and Sozin made it pretty clear that the reincarnation is basically immediately; the scene where Roku is overwhelmed by the volcanic gas and ash cuts straight to Aang's birth.

And I would imagine that each of the four nations have their own ways to identify the new Avatar- we saw the toys with Aang, and the monks announcing Roku... apparently the southern Water Tribe's Avatar identification method kinda got pre-empted by Korra herself. XD

Put a baby in a boat, set it off into waters with ice, if it survives it's the Avatar. Seems legit.
