One presumes that it's a perk gained from having a higher site status, such as being a moderator.

Moderators iirc have the ability to change their pretitles using the moderator controls to go above the regular character limit.

When I became a forum mod, CG edited my pretitle to what it is right now. Comfed is correct — we can change pretitles to be substantially longer than their limit from page=settings via the moderation tools.
