The IMK is part of the MME 00 system of personal equipment, in use by all deployable Themisi military personnel and many police officers. Considered a critical part of the deployable soldier's equipment, the IMK is designed to be both lightweight and unobtrusive. The pack is compatible with a range of load carrying equipment series, and easily modified should there be a change in medical protocols, or if equipment should need replacing. Whilst the container has remained the same since 2000, the equipment carried within has changed sequentially, with the current deployable standard being equipment Series 6.

As part of compulsory training for overseas deployable personnel, and for militia personnel at higher readiness, first aid is one of ten critical areas covered. The IMK as such is one of the critical pieces of equipment required. The aim is that the equipment within may be used by any soldier, or the soldier themselves, to prolong life until higher medical attention is sought. By distributing this to all soldiers, this allows critical care to be administered within seconds to minutes of traumatic injury, which has been proven to prevent a significant majority of avoidable combat deaths. Using simple equipment in a permissive environment, a first responder can prevent deaths that occur within minutes of injury from catastrophic bleeding and airway occlusion, and reduce the chance of death in transport from injuries to the lungs and minor bleeds.

The equipment in the current issue recognises this, and is presented in a logical way. On opening, the left hand side of the container holds a Combat First Response (CFR) Memory aide, a small card booklet clearly detailing the process of applying such aid in a flowchart format, so that all levels of soldiers are able to carry out first aid. This is made obvious in order to prompt soldiers to utilise the guidance provided. Included in this side are an indelible marker pan (black) and casualty marker card, to allow for multiple casualties to be marked for the purpose of triage, and for basic reports to be taken if necessary. A set of protective nitrile gloves (hypoallergenic) are provided in order to prevent exposure to infectious agents or bodily floods, and a chemical light is included to mark the position of the casualty. A small set of heavy duty clothing shears is included to allow exposure of wounds.
The second pouch of the kit contains equipment for dealing with catastrophic bleed and airway issues. A fast-application tourniquet is included to treat wounds. The current model included is the C.A.E. - III, a Themisi-manufactured appliance that utilises a velcro-elastic system, with external windlass, that can be applied by the injured or first responder in seconds. Included are two pressure bandages, for additional management of catastrophic and major bleeds.

A nasopharyngeal airway is included, to support the airway of semi-responsive and unresponsive casualties. The CHI Surgical NPA pack comes with a small sachet of surgical lubricant, in order to facilitate easy insertion of the NPA.

The third rear pouch of the kit contains a chest seal pack and gauze, in order to control open pneumothorax, and less serious bleeding from wounds.

A final small pouch holds space for controlled analgesia. The current issue utilises Fentanyl lozenge sticks for sub-buccal administration, with two units provided in sealed containers. Fentanyl was chosen due to numerous advantages over morphine when issued on the battlefield - namely its reduced potential for addiction, more rapid onset, and ability to be administered via the buccal route rather than an injection being necessary. Two are carried for self administration, with further doses administered after the hour window provided by a trained medic.

The container externally is zipper-sealed, opening to show two front pouches, and two rear pouches. The external zipper seal is water resistant when in place. Elasticated pockets internally hold the contents in place. Externally, MOLLE fabric loops are in place to allow attachment of additional pouches or panels, with a velcro area to allow attachment of personal details to the medical kit (e.g. allergy/blood group patches). On the rear, the pouch attaches via two straps to MOLLE systems used with MME 00 issue equipment onward, with the ability to clip into older LCE series equipment used pre-2000. The unit is designed to be carried either on belt-based load carrying equipment or body armour, but can be carried on backpacks as per unit doctrine and individual preference.

As with all MME 00 equipment, the IMK is provided in Olive Drab MME 00 fabric as standard, and as such is IRR, fire resistant, and water resistant. Lines produced for external nations are offered in other fabrics as requested.

The BIMK is a medical system produced for forces with basic medical training, as a personal medical kit. Originally a militia issue system, the BIMK has been phased out in favour of the IMK system in Themisi service, though is still in production for civil defence and private orders.

The BIMK is a cheap, simple system, holding basic components for administering first aid. In line with current guidance on pre-hospital care, the BIMK focuses on haemorrhage control, casualty marking and personal safety, in order to support a casualty and reduce mortality before their arrival at a medical station. With the theory that some states would save more life with a number of more basic, cost-effective kits rather than a few, expensive kits, the BIMK is sold to developing armed forces as an effective medical solution.

The container utilised is manufactured from MME-85 series material, with water-resistant, flame retardant and IRR properties. The material is somewhat heavier than contemporary fabrics, but is proven as robust and rip-resistant. A clip fastener is used to seal the single pouch, with additional internal velcro fastening to secure contents. The contents are held in two sealed waterproof pouch for sterility, water resistance and shelf life, the pouch being resealable with a basic fastener. The container attaches to load carrying systems with MOLLE-type fastening loops, and is also compatible with older LCE style fastenings of the 85 and 60 series. Internal eyelets on the base of the pouch allow water and dust to drain from inside.

Internally, the front divider of the pouch containers the tourniquet and pressure bandage, alongside gloves. This allows for rapid access to critical haemostasis aids, allowing control of bleeds rapidly, either from limbs or from open wounds.

The rear divider holds the second bandage, paperwork packs (Aide memoire + casualty card pack), and pain relief. An aide memoire allows for easy revision of medical procedure and drills by soldiers with limited medical training, whilst the casualty card pack allows for simple marking of the carrier as a casualty in a mass casualty event, or for rapid handover to medical crews.

Morphine autoinjection pens are the analgesia of choice, as a more cost-effective option to Fentanyl. Administered intramuscularly, two pens are provided for initial pain relief of up to two hours.

Additional space is included in the rear divider for additional medical equipment as necessary.

Recognising the need to promote self care amongst military personnel, all persons deploying on training exercises or operations are now issued a personal medical kit to reduce the burden on primary health care services, and to augment personal fighting capability. The pack is based on a simple formulary, applicable and useful for travellers and military personnel alike.

The PEMK is issued in a small zipped container, for carriage in a daysack or larger rucksack system. It is designed for non-emergency usage, for dealing with common complaints in primary medicine, and to assist in preventative medicine. The container is made from MME-00 olive green textile, and is flame retardant, IRR and water resistant. The container opens into two, with both sides holding their contents in zippable fabric mesh pockets. Contents are held within resealable transparent plastic bags, marked with their contents. The pouch has MOLLE loops on the exterior, allowing it to be attached to other equipment if necessary.

Medications for treatment of common conditions are held within the pack. For pain relief, Ibuprofen is utilised for muscular pains as an NSAID, exhibiting additional anti-inflammatory properties, with the lowest short-term risk profile of NSAID medicines. Ibuprofen sodium 200mg tablet formulation is used, and is included dispensed in a single bottle.
Paracetamol 500mg is included for other minor pains and fevers, for its low profile of side effects, and for its compatibility with NSAIDs for controlling moderate pain. 500mg tablet formulation is used, and is included dispensed in a single bottle.

For mild allergic conditions, such as hayfever, rash sinusitis, and irritation from chemical/pollen exposure, Cetirizine 10mg is included as an effective, non-drowsy medication. It may also be used for irritation from insect bites. 10mg tablet formulation is used, dispensed in a foil blister pack.
For bowel conditions, Loperamide 2mg is included to treat non-bloody diarrhoea, in foil blister packs in 2mg Capsule formulation. Macrogols sachets (assorted fruit flavours) are included for the safe treatment of constipation, and faecal disimpaction (under medical supervision). The formulation is for constitution in water, with 12g provided in a foil sachet. In cases of diarrhoea in the field, oral rehydration sachets containing electrolytes are included, to prevent electrolyte imbalances and dehydration-related illness. The formulation is for constitution in water, with 15g of salts, glucose and starch included per foil sachet.
For indigestion, sodium hydrogencarbonate anti-acid tablets are included, for short-term treatment of this condition. The drug is dispensed in fruit-flavoured tablets, in foil blister packs.

For wound treatment, a number of items are included. Melanin non-absorbant wound pads are included to protect wounds after cleaning, and can be cut to size with the scissors included. The pads are non-adhesive for wound protection. Strapping tape is included for both securing dressings, and for strapping feet/wrists for strains and blister prevention. Anti-septic wipes are included for wound cleaning purposes. A bandage pack containing a medium elasticated bandage and safety pins is included for supporting damaged joints, and protecting larger dressings. Barrier plasters are included for minor injuries, alongside gel hydrocolloid plasters for covering blisters, especially on feet.

For treating and protecting dry skin, from sun or chemical exposure, petroleum jelly is included. Finally, for disease prevention, a tick remover is included to remove ticks safely and early, reducing the risk of acquiring disease from these parasites.

A contents packing list is included as an insert, alongside the Personal Self Help Card. The card details what the kit contents are to be used for, and how to use them, alongside a brief guide of how to look after oneself, and when to seek medical attention. Medication instruction leaflets are included as per law for patient information, with medication administration instruction printed onto each medication bottle.

The PEMK-T insert is an additional pack of equipment for tropical climates, held within the normal PEMK. The PEMK has sufficient space for the additional medicines, which are held in their own separate resealable, waterproof bag.

The main addition is the course of anti-malarial tablets. As is standard, a combination tablet of 100mg Proguanil and 250mg Atovoquone is utilised. The once-daily nature of the tablet aids in easy taking of the course, alongside rapid induction of two days. The side effect profile is minimal with the tablet, and the combination of medicines prevents resistance forming to prophylaxis. Three months supply is included, with additional resupply as necessary.
A second option exists for anti-malarials. In nations where it is appropriate, a course of Mefloquine 250mg is included, taken once-weekly. This options is seen as more suitable for nations where budgeting is an issue, due to the cost of the first option anti-malarial. Downsides to the tablet include longer induction period (2.5 weeks), and greater incidence of side effects, both physical and mental.
The third option is the use of Chloroquine 310mg. Whilst effective in some parts of the world, Chloroquine is gradually being used less, due to the resistance seen (in some places, total) by the malarial parasite. Chloroquine is the cheapest option, with moderate induction period (1 week), and a similar incidence of side effects as Mefloquine.

Anti-worming tablets are included, both as prophylaxis in less developed areas, and to clear infestations, thus increasing fighting effectiveness. The combination therapy included utilises 16mg Ivermectin and 1.5g Praziquantel to clear a very broad spectrum of worm infestations, and prevent re-infection for up to 2 weeks after the course.

An additional tick remover tool is included, in case of loss of the initial tool. An antiseptic cream is included for preliminary treatment of skin irritation in tropical climates, acting both against fungal and bacterial skin infections that are common in such conditions. Povidone-iodine wound antiseptic is also included, for treating cuts more effectively than normal anti-septic wipes, thus reducing the chance of wound infection in such countries.

Two guides are included for tropical climates. A basic card identifying poisonous animals and plants in the area of operations is included, tailored for the nation in question. A generic cue card is also included as a reminder of how to prevent injury and illness in tropical climates, and when to seek help.

The Personal Combat Medical Set (PCMS) is a contract product to the Golden Throne, assembled as an urgent operational requirement for combat forces. As with all individual combat medical kits assembled by SWT, the aim of the equipment is to rapidly control and stabilise causalities on the battlefield, giving them the time needed to reach definitive medical care.

The PCMS is contained within a single, moderately sized pouch, with zipper and button fastening for closure and rapid access. The unit is made from heavy duty fabric as used in the MME-00 series of equipment, the 1500D durable weave being resistant to heat, flame and ripping. The fabric as provided is IRR, and water resistant. The pouch fastens to most load carrying equipment, with MOLLE-type rear fastening loops. A velcro patch is provided on the front for personal badge attachment (e.g. Allergy/Blood Group status), with an elasticated bungee web for holding additional equipment as desired.

The pouch itself opens up fully to reveal the interior compartments. Four internal compartments within are used to divide up equipment, with fine fabric mesh used to hold the items in place and simple zipper to open and close. An external zipper compartment is used to hold documentation and other useful administrative items. All zippers are water resistant when closed, with drainage eyelets provided on the base of the pouch.

The equipment chosen for the PCMS was included based around existing doctrine amongst Golden Throne forces, and current trends in traumatic medical care both there and in Themis. As such, the priority components within the internal pouch are the haemostatic products. Two tourniquets are included as standard for haemorrhage control within peripheral limbs, with a further two medium-sized pressure bandages included to support their action. For penetrating wounds, haemostatic gauze is included based upon combat evidence of effectiveness in controlling catastrophic haemorrhage.

For airway maintenance, two airway adjunct choices are chosen, with simple devices included as basic key interventions. NPA type airways are included, the pack containing a small sachet of surgical lubricant to aid insertion, for semi conscious casualties or casualties with jaw injuries. For the unconscious casualty, OPA type airways are included in two common sizes, for temporary airway protection. A face shield is provided to prevent cross contamination when performing artificial respirations.
Recognising common complicated injuries to include chest injuries such as pneumothorax from blasts and penetrating trauma, a chest seal pack and thoracentesis needle set are included to allow rapid decompression of suspected pneumothorax, and sealing of open chest wounds.

In patients suffering from blood loss, fluid resuscitation is provided for administration once haemostasis is achieved. Based upon advances in preservation technology, freeze-dried plasma is included as the resuscitation of choice. The plasma utilised is derived from donated blood stocks, where often a surplus of blood plasma prevails (red blood cells being a more often used component). Plasma provides an advantage over simple crystalloid fluid in having greater osmotic potential, reducing loss of fluid from vasculature to other tissue spaces, and thus maintaining greater circulatory volume. Additionally, the inclusion of clotting factors within plasma ensures that any clots formed during haemorrhage control remain more stable than if plain crystalloid were used. The plasma used is initially typed as universal plasma group (AB+), containing no antibodies to the most common 12 minor blood group antigens, and undergoes secondary screening for contamination from a standard panel of blood borne viruses or any bacterial/fungal/protozoal contamination. After freeze drying, the product is transferred to a semi-rigid plastic bottle in a sterile environment, and sealed under low vacuum. For reconstitution, 500ml of 0.9% normal saline in semi-rigid plastic bottle is provided with a blunt filling needle; after reconstitution, the product is safe to use within an hour.

In order to administer plasma for fluid resuscitation, an IV access kit is provided. The kit contains the basic equipment needed for venous peripheral catheterisation within a single bag:

The AVESS cannula system is widely used within Themisi health centres, with a sturdy high grip body allowing easy cannulation whilst wearing gloves or if fluid covered. A safety introducer prevents needlestick injury by automatically capping the metal needle for cannula introduction. Two ports built into the cannula allow for the administration of IV bolus injections whilst an infusion is running on the same line. The intravenous component of the line itself features an ultra-sonically applied nano-coating, inclusive of a hydrophobic/antithrombotic component to prevent clotting within the line and ease flow, and an antimicrobial/antiseptic coating to reduce the incidence of line infections or colonisation. A similar hydrophobic coating is applied to the inside of the IV giving set, ensuring flow of products given.

Additional items are included for completion of care. For pain relief, Fentanyl is included as a more effective, less addictive and more easily administered strong analgesic. The drug is administered in buccal form - the small lollipop is held under the tongue for absorption over five minutes, with a sugar-free sweetener added. The drug is supplied in sealed containers to monitor tampering, with two provided for up to two hours of pain relief. For exposure prevention, a foil blanket is included, to provide lightweight warming of a casualty, or to be used as a heat shade in the event of heat exposure. A rectally-administered combination of antibiotics is included for prophylaxis of wound infection; administered early, there is evidence that such prophylaxis can prevent wound infection or contamination. Rectal administration allows for the drug to be given even if IV access is not gained, with high absorption. Ampicillin, whilst covering common contamination from skin commensals (e.g. Staphylococcus, Streptococcus), is contra-indicated in true penicillin allergy. As such, it may be omitted from the packs of those with penicillin allergies after issue. Metronidazole as included provides cover against anaerobic bacteria and organisms often associated with soil contamination in wounds (e.g. mine/blast injuries), and is associated with fewer instances of allergic reaction or side effects.

For medical admin, a number of items are included to ease casualty handling. Casualty triage card is included in the medical kit for the purpose of marking a casualty's triage status, and recording pertinent information for the medical handover, alongside a MIST slate with permanent marker for recording medical information for handover and evacuation requests. Clothing shears are included for rapid removal of clothes over injured area, with gloves for personal protection from contaminated bodily fluids. For marking a casualty in the dark, a red chem light is included for easy location of injured persons. As a final inclusion, an aide memoire booklet is provided on waterproof paper, to provide an easy to follow flowchart guide for soldiers in administering emergency care. The GT aide memoire is printed as per current doctrine in the Golden Throne forces in Dienstadi.
