The costs per flight hour typically quoted (F-22 needs twice as much maintenance as a F-15 lol) are operating or ownership costs per hour, basically entire program costs divided by flight hours. A small fleet flying less total hours will have a higher operational CPFH - this is the reason combat-capable trainers typically have CPFHs about 50% higher than single-seaters.
In 2011, for example, the USAF's small remaining fleet of F-16As logged 62 hours of flight time at a cost of $227,559 per hour. The year before that they flew 112 hours at a cost of $132,204 per hour, and in 2009 502 hours at $26,432 per hour.
Operating CPFHs are highly variable from year to year depending on the number of hours flown and costs not directly related to maintenance (such as modifications or crashes), and small fleets exacerbate that. In 2010 F-22s flew 24,817 hours at $60,682 per hour, but in 2011 only 15,578 hours. Predictably, the CPFH went up to $111,779. In 2012 it was back to 26,677 hours at $60,507 per hour. F-15s are about $30-40,000 depending on type - they also log four times the fleet hours.
In extreme cases this method of accounting can lead to costs per hour being millions of dollar higher from one year to another.
Maintenance and consumables are variable costs per hour - F-15s are about $17,000 and F-22s $19,000. This still isn't great, since one of the ATF goals was to reduce maintenance costs with advanced diagnostics.
