Spoilers, as I know, should always be on the outside. Please correct me if I am wrong, but you'd never italicize or align something out of those boundaries. Spoilers are like the envelope, and everything inside must be sealed by it.

Basically this- proper coding protocol has you close out tags in the same order they were opened, as Punk shows with the Hey/Yo example. Certain codes, such as italics, bold, or underline are relatively forgiving; the problem tags that NEED to be nested properly to avoid display breakage are spoilers, quotes, boxes, align, and tables. Some other combinations might also screw up the display, but those are the most common sources of the broken thread problem.
